    Incident Report

    A structured record of an event that requires documentation, escalation, or follow-up.

    An incident report captures what happened, when, where, who was involved, and what was done. Well-written incident reports are the single most-scrutinized artifact when a client evaluates a security vendor after something goes wrong. They must be accurate, timestamped, and defensible.
